    The Home Health Manager position is open in our Upland location. This is a Full-time position during our dayshift.

    Requirements of the Home Health Manager:
    * Current RN license in State of CA
    * A minimum of one year nursing experience prior to employment in a professional care setting.
    * Minimum 2-3 years supervisory experience in community, home health or institutional direct patient care.


    Home Health Manager Duties Include:
    * Responsible for oversight of assigned Home Health office location.
    * Responsible for oversight of nursing to include specialty nursing, therapists, (PT, OT, ST), SW, CHHA. Maintains responsibility for their work assignments. Promotes and provides for the highest quality of care.
    * Works closely with the Clinical Systems Supervisor on oversight of office staff and related activities.
    * Performs timely performance evaluations in conjunction with field supervisory visits with all nursing personnel.
    * Coordinates and schedules 24 hour and weekend nursing coverage.
    * Performs corrective action in an appropriate and timely manner following VNA Policies & Procedures.
    * Responsible for oversight of other paraprofessional staff (PT, OT, ST, RD)
    * In conjunction with the Rehab Supervisor ensures the highest quality of care is provided.
    * Provides insight to Rehab Supervisor for performance evaluations.
    * Responsible for oversight of paraprofessional (CHHA) staff on provision of quality care, timely performance evaluations and field supervision.
    * Monitors staff workload and productivity for both quality and quantity.
    * Determines either directly or indirectly, appropriate utilization of services (for all payors).
    * Coordinates review of documentation. Oversees Utilization Review staff for designated office.
    * Works closely with Director of Performance Improvement and Director of Home Health Clinical Operations to ensure care and services are meeting regulatory guidelines and meeting quality outcomes.
    * Works closely with Director of Hospice, Hospice Manager and VNA Private Duty Manager.
    * Utilizes agency manuals when necessary, i.e. Administrative Policies & Procedures and Safety; recommends updates when appropriate. Provides necessary education to staff members.
    * Assists in resolving billing problems and ensuring that staff understand and follow authorization request process.
    * Actively participates in agency meetings/committees. Serves on outside committees as assigned.
    * Works closely with Business Development in community education and marketing. Develop relationships with community members to assist agency in its post-acute care partnerships.
