  • Licensed Vocational Nurse
  • 1 year of experience required
  • Position qualifications

    1 Graduate of an accredited school of nursing program required.

    2 Current Licensed Vocational nurse registration with the state of California.

    3 At lease one year experience in Hospice of related health care field.

    4Must have at all times a satisfactory DMV driving record per VNA insurance company criteria.

    5 Current CA drivers license and CA liability insurance.

    6 Current CPR Card

    PHYSICAL REQUIREMENTS

    Visual and hearing ability sufficient to comprehend written and verbal communication

    Ability to lift up to 50 pounds

    Ability to preform tasks involving physical activity, which may include extensive bending and standing.

    Ability to deal effectively with stress.

    PREFORMANCE RESPONSIBILITES AND STANDARDS

    1. Ensures quality and safe delivery of hospice services

    2 Implements current nursing practices and provides direct hands on care within the scope of practice.

    3 Implements effective written/verbal communication with Hospice team members. Documents in a clear concise manner.

    4 Uses Equipment and supplies effectively and efficiently

    5 Participates in the Interdisciplinary team meeting to share information regarding patients/families, ensuring continuity of care. Notifies the team of any changes in condition.

    6.Obtains orders as needed to promote comfort and quality of life. Keeps Case Manager informed of all changes in condition and orders obtained.

    7 Promotes education for clients, client families and the community.

    8 Provides Hospice program explanation as requested.

    9 Participates in professional growth and development.

    10 Demonstrates a positive, flexible, customer comes first attitude.

    11. Participates in QAPI program.

    12. Maintains patient/family confidentiality according to HIPAA regulations.

    13.Adheres to agency policy and procedure

    14 All other duties assigned
